描述.In June 2021, Doctor and Patient decided that time had come to surgically implant two six-inch-long metal alloy spikes all the way through Ben’s brain. It was felt that the medications Ben was taking to control his Parkinson’s disease had become unmanageable. .Back then, Ben was taking about 20 different pills a day. Each pill, if it absorbed properly, would activate the dopamine pathways in his brain and induce uncontrollable writhing movements that would last for about an hour. He would then get about 20 minutes where he’d feel somewhat normal before the slowness and tremor kicked in again. So, he’d take another pill and the cycle would repeat..After months of adjusting his medication and finding just the right settings on his deep brain stimulator, it was decided, for the first time ever in a clinical trial in North America, to flip on the adaptive settings..This is the story of how that decision was made and what happened next..

The Cards We Are Dealt,Here we focus on the surgical options available to patients with Parkinson’s who are considering getting one of the advanced therapies. We list the advantages and disadvantages of each and try to explain how none are a one size fits all solution to the many problems these diseases present. We end by talking about why we chose DBS for Ben.

The Future of Cell Replacement Therapies,Some preamble as well as some allusions to Ben’s first book written with Dr. Alberto Espay titled . before an article Ben and Gerold Riempp wrote about CRT and why they were both strongly thinking about going forward with it at the time.

Time, Healing and Patience,We give more detail about the programming and talk about why it takes so long and what could be done to potentially speed this up. We also talk about why it is so important that the patient be as patient as possible while elucidating some of the things they may experience during.

,What It’s Like Letting an Algorithm Run Your Brain,More depth is given about adaptive DBS, about how we got to that point and what it felt like after we turned it on in Ben. We also go over more of the history of DBS and such adaptive devices, as well as a discussion on side effects and how he feels today. We end the chapter by giving a glimpse into the future of this technology.
