Hemodynamic and Ventricular Dynamic Investigations of Nitrate Tolerancetolerance exists with regard to a particular single effect of the drug or for the therapeutically desirable overall effect. Self-tolerance is the condition whereby a reduction in efficacy develops for the same substance, whereas cross-tolerance is when another similarly acting substance is ineffective.

Long-Term Isosorbide Dinitrate Therapy for Pulmonary Hypertension in Patients with Chronic Obstructiapers confirmed the salutary effect of ISDN on pulmonary artery pressure (4, 8–10, 12, 14). It was generally administered only for a brief period. Both observed no favorable effect in long-term therapy.

Lack of Tolerance Development to the Hemodynamic Effects of Nitrates in Patients with Chronic Congesilure, while tolerance to nitrates as antianginal agents has been suggested (8–10). Furthermore, the venodilating effect of nitrates may become blunted with time (11). It is therefore extremely important to establish long-term hemodynamic efficacy of nitrates if they are to be important in the treatment of chronic congestive heart failure.

The main problem in chronic treatment with nitrates is whether tolerance occurs or not. We have carried out three studies and started a fourth with a view to providing an answer to this question.
