描述Chronic obstructive pulmonary disease (COPD) is an increasingly important health problem world-wide. Whereas most other leading diseases have shown a decline in the past decades, COPD morbidity and mortality is on a steady increase. Exacerbations are usually defined as an increase in cough, a change in the colour or quantity of sputum, and a worsening dyspnea. The role of bacterial infections and the efficacy of antimicrobial therapy in acute exacerbations of chronic bronchitis (AECD) is still controversial. Exacerbations of chronic bronchitis are a common occurrence in clinical practice and are a leading cause of antibiotic prescription among respiratory infections. It is still uncertain whether each new exacerbation may deteriorate the natural history of chronic bronchitis. Undoubtedly, every episode includes a temporary worsening in lung function and may therefore pose the threat of respiratory failure or death in more severely obstructed patients.

Designing Future Clinical Trials for Acute Exacerbations of Chronic Bronchitis,(AECB) have been modelled on pneumonia studies. Research in both asthma and pneumonia has been the inspiration of subsequent studies in chronic bronchitis in many cases. Furthermore, a specific approach to the disease which takes into account its unique characteristics has seldom been used by researchers.

Acute Exacerbations of COPD: Aetiology and Antibiotic Resistance, but their cause may be difficult to identify and might include viral infections, environmental pollutants, allergic responses and bacterial infections. The cause may be multifactorial, so that viral infection or level of air pollution may exacerbate bronchitis, which in turn may predispose to secondary bacterial infections.

Infection and Antibiotic Treatment in Acute Exacerbations of Chronic Bronchitis,s (AECB) is an abuse of antibiotics, and can contribute to antimicrobial resistance. However, a recent meta-analysis has concluded that antibiotics are of benefit, and available data show that this benefit is most clear in patients who have at least two of the three cardinal symptoms of exacerbation [2].

Acute Exacerbations of Chronic Bronchitis: In Search of a Definition,“They [exacerbations] are extremely difficult to grade and undoubtedly some patients do want to please a doctor who has shown more than usual interest in their tiresome and commonplace disease” [1]..“The value of future work ... would be increased if agreement could be reached on the definition of an exacerbation” [2].
