What can we learn from classical operative techniques for valvular balloon dilatation?gery, and valvular balloon dilatation might have a lower mortality and morbidity rate, and repeat dilatations are thought to be easier to perform. In Table 1, an attempt is made to compare these postulated benefits with those of “classic” cardiac surgical techniques which, in general, provide well established and documented results.

Low-speed rotational angioplasty — clinical results in 53 patients with chronic occlusions8, 9, 11) can be reopened successfully only in about 50%–60% of cases. The chance of recanalization decreases with the duration of occlusion and the amount of calcification. The new technique called low speed rotational angioplasty (6, 14, 15) uses a very flexible, blunt, rotating catheter.
